One of the security gaurds and couple of employees at the plant that made the game pieces got busted stealing all the big winning pieces. They finally got caught when McDs didn't tell the company what part of the country the winning pieces were supposed to go to. Prior to that, the theives had been sending the pieces to family members in the part of the country that winning pieces were supposed to appear.

With the advent of internet registration of contest, fraud like this can't happen anymore, but I am sure that there are some enterprising crackers trying to generate key code programs to to fingure what the winning codes. For those that don't know what a key code program is, it is a program that generates the apparent random letters and numbers you see. They have already figured out how to generate codes to register pirated copies of computer games. It certainly wouldn't take much for them to cross it over to contest key codes.
