Are you sure he didn't mean roughly 5 of each player is known (which seems kind of high, since I doubt that many T4's are around)? I can't think of any set where we know exactly how many of any card was produced.

Mark mentioned seeing ads from a later time frame that required only 30(?), then 20(?) coupons to obtain a premium. Did they do this to get rid of left over premiums or because they weren't getting enough requests for premiums? Also, cards of the same player have picture cropping variations. This would lead one to believe that pictures were printed as requested.
